Reporting a Lost or Stolen Travel Card

This page provides information on how to report a lost or stolen Travel Card.

If your card is lost or stolen:

  1. Contact U.S. Bank Customer Service Office at (800) 344-5696.
  2. Report a stolen card to the local law enforcement agency.
  3. Contact your fiscal officer and email cardprogramhelp@ucdavis.edu indicating how the card or account number was compromised. Be sure to include the last 4 digits of the compromised card number in the email.

Note: The UC Davis Police should be contacted if a Travel Card is stolen on campus or used fraudulently on campus. In those cases, the cardholder should file a report with the police as soon as possible, in addition to completing the other steps outlined above.
