New Streamlined Covered Services Carve Out Request Form

Beginning Monday, March 31, a new webform will be used to make requests for Carve Outs to UC’s Covered Services policy. 

The form requires much of the same information previously requested, but is simplified in the following ways:

  • A single form is used, regardless of the estimated cost of the proposed service
  • Data is entered into a more user-friendly webform
  • Easy-to-use dropdown menus are used for several questions
  • Several form fields now include links to helpful information that can assist in providing an accurate response to the question

One additional question has been added to the request form. UC employees cannot be demoted, laid off, or undergo an involuntary reduction in time as the result of a contract for covered services. Initiators will be required to answer “Yes” or “No” to, “Are you aware of any existing employees who will be displaced as a result of this contract?”  Please note that if the answer is "Yes," the request will be denied.

The form routing has not changed. As with the previous process, DocuSign will route the request for the necessary signatures and the initiator will receive a copy when the process is complete. 

The new form is now available on the Supply Chain Management All Forms page, and is available for use along with the current forms. Requests in route using the current form will be processed as usual; any requests initiated on or after March 31 are to be processed on the new form.
