New Travel Card Application Now Available

Applications for the new Corporate Liability cards are now available.

Please note: in response to suspended campus operations, the estimated date for distributing the cards has been pushed to late May. For those who have not yet taken the training, please do so as soon as possible to prevent delays in receiving your new card. Travel Card Corporate Liability Training is about 30-minutes long and completion is required before applying for the new card.

Campus card applicants (including School of Medicine and School of Nursing):

The application for the new card for UC Davis campus employees is now available online. Please complete and return the application as soon as possible to prevent any possible delays. Please note: A campus applicant's fiscal officer must take the training before the employee can apply for a new travel card.

There are a few pieces of information campus employees will need to complete the new card application:

  • Your Fiscal Officer’s campus user ID (not password).
  • The default chart and account you want to associate with the card. Check with your fiscal officer to confirm this information.
  • Your department head’s first and last name.

After completing the application, you will receive a link from DocuSign to sign the application. The application will then route to your department head for signature and to your fiscal officer for signature. The application will then route back to the Card Program for processing. You will receive updates on the status of your application throughout this process.

UC Davis Medical Center applicants

The application for UC Davis Medical Center employees can be found here. Please complete and return the application as soon as possible to prevent any possible delays.

After completing the application, you will receive a link from DocuSign to sign the application. The application will then route to your department head for signature. The application will then route back to the Card Program for processing. You will receive updates on the status of your application throughout this process.

Card Distribution

We expect to begin distributing the new cards in late May. The old card will be cancelled approximately two weeks after the new one is issued and activated. Once you have received the new card, you will be asked to complete a brief online questionnaire confirming that you have received it. At that time, please activate the new card and then destroy the old card. After our initial deployment of cards, we will contact all remaining cardholders who have not completed the training or application. We will provide them a short grace period to complete the training and apply for the new cards, after which their existing cards will be deactivated.
