AggieSurplus Service Request Form Complete the AggieSurplus and Services form for all AggieSurplus requests.
Decontamination Form (PDF) Review the information on the Environmental Health & Safety website first.  Used when items contain, or were used to store, hazardous materials. Form is completed with the help of your department’s EH&S Specialist and needs to be attached to items prior to pick-up.  For the safety of our staff, items without the completed form attached will not be transported for removal.
Surplus Property Form Complete the online Surplus Property Form if you are a department that wishes to consign or sell an item with a value exceeding $250.00.  It is used to sell an item through AggieSurplus that will remain at the department location while being sold. Contact AggieSurplus to determine fair market value prior to completing this form. Revenue sharing only occurs when items sell for greater than $250. With the exception of ‘lot sales’, only one item per form should be submitted.
Non-Profit Application Form (PDF) Accredited schools and non-profit organizations can complete to be considered for potential donations through AggieSurplus.  Please email completed form to aggiesurplus@ucdavis.edu
Pick Up Request (Spreadsheet) This form is used to identify what items are being requested for pick-up by AggieSurplus and Services.
Vehicle Form (PDF) This form is to be used when disposing of/selling a vehicle.
